A 2026 review of 23 randomised trials found berberine lowered weight by about 0.9 kg more than control. An earlier review of 12 trials put it at about 2.1 kg. In the main semaglutide trial, adults lost 14.9% of body weight over 68 weeks versus 2.4% on placebo, a 12.4-point difference. These are trials, but the berberine studies are small and the comparison is across separate trials, not head to head.
Take care: Nausea and stomach pain are common, and it is not safe in pregnancy, breastfeeding or for infants.
Do not swap a prescribed weight-loss or diabetes medicine for berberine; ask a pharmacist before combining it with other drugs.
